LDSF Sponsorship Form

Are you a living donor, transplant recipient, or transplant center personnel looking for financial sponsorship for an event? The Living Donor Support Foundation (LDSF) may be interested in sponsoring you.

The Living Donor Support Foundation will consider applications for event sponsorship from living donors, transplant recipients, and transplant center personnel for a wide variety of events that focus on raising awareness for living kidney and/or liver donation.

Please complete the form below to be considered for sponsorship. Applications must be submitted at least 60 days before the event start date. LDSF requires at least 30 days from receipt of an application to make a sponsorship decision. If approved, we request at least 30 days before the event to promote the sponsored individual and their event.

LDSF Sponsorship Terms and Conditions

The Undersigned recipient of an LDSF Sponsorship (“You” or “Sponsored Participant”) hereby agrees to all of the following Living Donor Support Foundation (“LDSF”) Sponsorship Terms and Conditions in conjunction with an event, including but not limited to a competitive athletic event (“Event”).

  • 1. SPONSORSHIP APPROVAL AND PAYMENT. Submission of an application does not guarantee sponsorship. If you are approved for an LDSF Sponsorship, LDSF will determine the amount, timing, and method of sponsorship payment or reimbursement at its discretion and will communicate those details to you, “Sponsorship Award”.
  • 2. FUNDRAISING RESTRICTIONS. You may not utilize any other peer-to-peer fundraising mechanisms, including but not limited to GoFundMe, in connection with your participation in the Event or your LDSF Sponsorship without LDSF’s prior written consent.
  • 3. APPAREL, LOGOS, AND BRANDING. You shall not utilize, wear, display, promote, or otherwise feature, whether on apparel, equipment, signage, social media, marketing materials, or in any other manner in connection with the Event or your LDSF Sponsorship, any logo, trademark, trade name, branding, or other identifying material of an organization, company, program, or initiative that is not part of the LDSF-sponsored or approved network of organizations, without LDSF’s prior written consent. For purposes of this Agreement, the LDSF-sponsored or approved network of organizations, companies, programs, and initiatives includes, but is not limited to, LDSF, National Kidney Registry, National Liver Registry, Donor Games, ImmunoFree, and any other affiliated, partner, or related organization, company, program, initiative, or brand designated by LDSF. The inclusion of these entities is intended to ensure that the apparel and logo restrictions apply consistently to the organizations and brands that operate under, support, or are aligned with the broader mission and objectives of LDSF. You shall not use, publish, reproduce, modify, or distribute the LDSF name, logo, trademarks, trade names, or other intellectual property owned or controlled by LDSF, or any similar names, trademarks, or trade names, in any marketing, advertising, promotional, fundraising, or other materials without LDSF’s prior written consent. This Agreement constitutes LDSF’s approval to use the LDSF name and logo solely in connection with the Event and the LDSF Sponsorship, subject to any branding guidelines provided by LDSF. You agree to comply with all reasonable LDSF requirements regarding the placement, size, appearance, and use of LDSF branding and the branding of LDSF-affiliated or approved organizations on apparel, equipment, signage, social media, and other promotional materials associated with the Event.
  • 4. PUBLICITY, SOCIAL MEDIA, AND LIKENESS. You shall provide LDSF with three (3) paragraphs of biographical information, no fewer than two (2) general photographs of you, and no fewer than five (5) professionally captured photographs of you for each day of the Event. LDSF reserves the right to request specific photographs of you during or in connection with the Event, which you shall provide within three (3) days of completion of the Event. You shall also provide LDSF with any and all raw video footage taken of you within three (3) days of LDSF’s request. You grant LDSF, and their affiliates, partner, or related organizations, a perpetual, worldwide, royalty-free, transferable, non-exclusive license to use, reproduce, publish, distribute, display, modify, and create derivative works from your name, image, likeness, biographical information, photographs, and videos for promotional, educational, fundraising, and other business purposes. You may use photographs and videos posted to your social media outlets during the Event, and during the thirty (30) days before and thirty (30) days after the Event, provided that you properly tag LDSF in such posts when the content relates to LDSF, the Event, or the LDSF Sponsorship. When posting about LDSF, the Event, or the LDSF Sponsorship on social media or other public platforms, you agree to clearly disclose your sponsorship relationship with LDSF in accordance with applicable laws, regulations, and guidelines. LDSF may request removal of content that LDSF reasonably determines is inconsistent with its mission, brand standards, sponsorship requirements, or applicable law.
  • 5. MEDIA AND PRESS. In the event you are contacted by the press or media regarding the Event, LDSF, or your LDSF Sponsorship, you may disclose the nature of your sponsorship relationship with LDSF. You shall provide LDSF with a copy or link to any published media coverage, including written, photographic, video, audio, or other media, within three (3) days of publication or receipt, if reasonably available to you.
  • 6. NON-COMPETE AND MONETARY PENALTY. During the period beginning upon your execution of this Agreement, and ending one (1) year after the Event, you shall not knowingly participate in, promote, represent, wear the apparel of, display the logos or branding of, or otherwise publicly endorse any outside organization, company, program, initiative, or brand that competes with or conflicts with the mission, sponsorship, or branding requirements of LDSF and their affiliates, partner, or related organizations, including but not limited to, LDSF, National Kidney Registry, National Liver Registry, Donor Games, ImmunoFree, in connection with an athletic event, fundraising activity, social media campaign, promotional appearance, or other activity that is reasonably likely to conflict with or diminish the value of the LDSF Sponsorship, without LDSF’s prior written consent.
  • 7. NON-COMPETE AND MONETARY PENALTY. During the period beginning upon your execution of this Agreement, and ending one (1) year after the Event, you shall not knowingly participate in, promote, represent, wear the apparel of, display the logos or branding of, or otherwise publicly endorse any outside organization, company, program, initiative, or brand that competes with or conflicts with the mission, sponsorship, or branding requirements of LDSF and their affiliates, partner, or related organizations, including but not limited to, LDSF, National Kidney Registry, National Liver Registry, Donor Games, ImmunoFree, in connection with an athletic event, fundraising activity, social media campaign, promotional appearance, or other activity that is reasonably likely to conflict with or diminish the value of the LDSF Sponsorship, without LDSF’s prior written consent.
  • 8. ASSUMPTION OF RISK AND RELEASE. Participation in athletic events involves inherent risks, including serious injury, disability, and death. You voluntarily assume all risks associated with participation in the Event. You acknowledge that LDSF does not organize, operate, or control the Event and is not responsible for the Event’s safety, conditions, or operations. To the fullest extent permitted by law, you release, waive, and hold harmless LDSF and its officers, directors, employees, and agents from any claims arising from your participation in the Event, except to the extent caused by LDSF’s gross negligence or willful misconduct.
  • 9. NO EMPLOYMENT RELATIONSHIP. Nothing in this Agreement creates any employment, agency, partnership, or joint venture relationship between you and LDSF. If you are a student-athlete, you are responsible for ensuring that participation in the LDSF Sponsorship complies with your school’s policies and any applicable athletic rules. By accepting an LDSF Sponsorship, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions.
